Minimum to Midpoint Pay Rate: $27.57 - $35.84 / hour
Master's in Social Work Needed (MSW Degree)
Home Health or Hospice experience desired but not required
This position visits patients in the home.
Responsible for the delivery of services to the Healthy Start client population as outlined in the Healthy Start guidelines. The Healthy Start Social Worker facilitates services across a continuum through collaboration with community resources, the Healthy Start team, and other professional disciplines to ensure effective and efficient delivery of Healthy Start services and the achievement of desired client outcomes. Supports the organization vision and mission. Uses knowledge of patient's age and cultural diversity into the delivery of services rendered. Contributes to the success of the Healthy Start team through excellence in customer service and performance improvement techniques that demonstrate positive outcomes of said services. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start /Home Health Team, and are expected in this role. Duties will include participation in events sponsored by Healthy Start, as well as covering team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our client caseload and the needs of the organization. Scope of Duties: Counseling the client and family in meeting related needs. Initiating appropriate preventive procedures Coordination of services Referral to other services as needed Informing the physician and other staff of changes in the client's needs (Healthy start) Supervising paraprofessionals providing services Participating in in-service programs
This is a driving role.
Education: Masters degree in social work required.
Experience: Minimum of 1 year of social work experience in medical or healthcare setting.
